How to become a Provider

Complete online enrollment
- Create an account and write down your username, password, and answers to the security questions. All three are case-sensitive and must be re-entered to watch the videos.
- Watch the mandatory videos.
- Electronically sign forms SOC 426 and SOC 846.
- Schedule an Orientation appointment.
Orientations are offered in English, Spanish, Chinese, and Russian. For help with scheduling an appointment or enrollment, call (415) 557-6200.
Are you a former IHSS Provider? Call (415) 557-6200 or email ihsspaymentunits@sfgov.org to find out if your Provider status is still active.

Attend 2-hour, group Orientation in San Francisco
Arrive 15 minutes early for your Orientation appointment at 77 Otis Street with:
- Valid state or U.S. government-issued photo ID
- Original Social Security card
- A Work Authorization (required only if your Social Security card states "Valid for work only with DHS or INS authorization")
- Completed the IHSS Provider Packet (including SOC 426A). Complete the packet with your IHSS Recipient, if you know the Recipient: English | Español | 中文 | русский | Filipino | Tiếng Việt

Get background check
Use the Live Scan form you received at Orientation to make a fingerprinting appointment with the San Francisco IHSS Public Authority.
Keep a copy of the Live Scan form as proof of completion.
