Disability + Aging Services Health Promotion
These two programs offer exercise classes for older adults and adults with disabilities. The program helps participants remain healthy and independent while increasing their energy and improving their wellbeing and sense of community. The classes include cardiovascular movement and strengthening of balance and flexibility.
Classes are offered by our community partner Onlok.

Older adults (60+) or adults with a disability who live in San Francisco.

Chronic Disease Self-Management is a free, six-week workshop developed by Stanford to help older adults self-manage their ongoing chronic health conditions. The workshop series focuses on self-management skills, including problem-solving, action planning, healthy eating, behavior modeling, setting and reaching goals.
Workshops are offered by our community partner Onlok and meet once a week for 2.5 hours. Each series is facilitated by two trained health educators and conducted in different languages.
Who is eligible
Any older adult who is living with an ongoing health condition. Caregivers, family, and friends caring for an older adult with an ongoing health condition are also eligible.

This free, six-week workshop developed by Stanford helps adults self-manage their pain. The workshop series dives into the Practicing Moving Easy program of gentle exercises, using mind-body tools, understanding medication usage, healthy eating, preventing falls, and more.

Diabetes Empower Education Program (DEEP) is a free, six-week workshop to help older adults with diabetes learn more about diabetes, self-manage their condition, and maintain a healthy, active lifestyle.
This workshop is offered by our community partner, Onlok, and meets once a week for two hours. The workshop is facilitated by a health educator and developed by the Midwest Latino Health Research Training and Policy Center at the University of Illinois at Chicago.
Who is eligible
Any older adult who is diabetic or pre-diabetic. Caregivers, family, and friends caring for an older adult with diabetes are also eligible.

HICAP is a volunteer program that helps Medicare recipients, their family, and caregivers understand their health insurance benefits, options, and rights. HICAP offers free, unbiased, confidential, and personal assistance from health insurance counselors registered by the California Department of Aging.
We offer help with Medicare, Medicare supplement policies, Health Maintenance Organizations (HMOs), and long-term care insurance. We can also help you with filing and preparing insurance claims and Medicare appeals, if your claim has been denied.
